One way to improve sight reading is getting a teacher who can coach you on books like Robert Starer's Rhythmic Training and Ottman's Music for Sight Singing.
Most of our sight reading problems are rhythmic rather than pitch related, but having a good sense of what it should sound like will improve your sight reading, and the beauty is all this work is done off the horn, so yo don't have to blow your chops on it.
Not that there's anything wrong with that.
Man, if only I was this smart when I was 15.
