"Cousins" for Cornet and Trombone

Hi all,
Does anyone know how I might obtain a copy of "Cousins" for Cornet and Trombone, by Clarke? It's on Gerard Schwarz's Cornet Favorites CD, and I would really like to play it. Thanks.

Just a note....if you try to look up Portland Sheet Music Service you may have some difficulty.

The actual name is Sheet Music Service.

Great group to work with. Nice to be able to actually speak with a real person who is knowledgeable and helpful.
